Permethrin Lotion is a topical medication used to treat head lice by paralyzing and killing the lice and their eggs. It is available over-the-counter in a 1% concentration (e.g., Nix®) and typically requires one or two applications to be effective.

Ingredients of Permethrin lotion:

The primary active ingredient is permethrin (1% concentration for the lotion used for head lice). 

Inactive ingredients in brand-name (e.g., Nix®) or generic formulations may include:

  • Isopropyl alcohol
  • Methylparaben
  • Propylparaben
  • Butylated hydroxytoluene
  • Carbomer homopolymer type B
  • Fractionated coconut oil
  • Glycerin
  • Glyceryl monostearate
  • Lanolin alcohols
  • Mineral oil
  • Polyoxyethylene cetyl ethers
  • Purified water
  • Sodium hydroxide
  • Formaldehyde (as a preservative in some 5% cream formulations). 

Usage Instructions of Permethrin lotion:

Follow the directions on the product label or your doctor's advice carefully: 

  • Wash hair with a regular shampoo (do not use a conditioner or a shampoo/conditioner combination product immediately before use) and rinse thoroughly.
  • Towel dry the hair until it is only damp.
  • Shake the permethrin lotion well to mix the medication evenly.
  • Applythe lotion to the hair and scalp, making sure to cover the areas behind the ears and at the back of the neck.
  • Leave the lotion on the hair and scalp for 10 minutes.
  • Rinsethe hair and scalp with warm water over a sink (avoid using a shower/bathtub to limit body exposure).
  • Towel dry hair and use a fine-toothed comb to remove any dead lice and nits.
  • Wash hands carefully after the application and rinsing steps.
  • A second treatment may be necessary if live lice are seen 7 days or more after the initial treatment. 

Side Effects of Permethrin lotion:

Most side effects are generally mild and temporary: 

  • Burning, stinging, or tingling sensation on the skin or scalp.
  • Itching or rash of the skin/scalp area (this may persist for a few weeks due to an allergic reaction to dead mites/lice and is not necessarily a sign of treatment failure).
  • Redness of skin or scalp.
  • Numbness or mild skin irritation.
  • Dryness or peeling of the skin (in some combination products).
